Impaired exercise response and other residua of pulmonary stenosis after valvotomy.

چکیده

The operation of pulmonary valvotomy has been so brilliantly successful that its failure to achieve an ideal end result in certain cases deserves comment. In this report, some residual effects of isolated pulmonary stenosis that may persist after complete valvotomy will be considered. It will be seen that all of these are the result of very severe stenosis or of long-delayed relief of stenosis of less severe degree. They constitute, therefore, a plea for early recognition of pulmonary stenosis and accurate assessment of its severity, so that operation, where needed, may not be unwarrantably delayed. The conditions to be considered fall into two groups, each of which will be presented and discussed separately. Group I. Results of right ventricular myocardial fibrosis: (A) impaired response to exercise, and (B) persistence of reversed interatrial shunt. Group II. Results of persistent right ventricular hypertrophy.

Pulmonary valvotomv PULMONARY VALVULAR STENOSIS

Twenty children with pulmonary valvular stenosis were studied by cardiac catheterization both at rest and during submaximal supine exercise, prior to and following pulmonary valvotomy. The resting and exercise states were comparable preoperatively and postoperatively since cardiac output, heart rate, A V 02 difference and oxygen consumption were similar. Evidence of improved cardiac function wa...

Haemodynamic effects of exercise in isolated pulmonary stenosis before and after surgery.

Surgical treatment of pulmonary stenosis has frequently been evaluated by the reduction of pressure gradient across the valve. In only a few patients, however, have exercise tolerance and haemodynamic response to muscular work been reported both before and after operation. Johnson (1962) compared the findings in 7 patients after pulmonary valvotomy with those in 8 patients without operation. A ...

Objective: Valvotomy and the autograft procedure are the most common surgical treatment options for children with valvular aortic stenosis. We evaluated the results of these surgical procedures in our institution. Methods: Retrospective analysis was done of all patients presenting with aortic stenosis and operated upon before the age of 18. In 11 patients a valvotomy was performed and in 36 an ...

Hemodynamic abnormalities in response to supine exercise in patients after operative correction of tetrad of Fallot after early childhood.

The exercise hemodynamic values in two groups of patients with repaired tetrad of Fallot (eight patients with some residual pulmonary insufficiency and seven patients without insufficiency) were compared with values in seven patients with trivial pulmonary stenosis who had not been operated on. The patients with tetrad of Fallot underwent surgery after age 8 years and all had a good hemodynamic...

Balloon valvotomy for critical stenosis or atresia of pulmonary valve in newborns.

OBJECTIVES Percutaneous balloon valvotomy was studied retrospectively in newborns with critical pulmonary valve stenosis or atresia to assess its potential role as an alternative therapy to operation. BACKGROUND Severe right ventricular outflow tract obstructions are life-threatening conditions requiring prostaglandin infusion immediately after birth and then relief of the valvular obstructio...
